Facility Evaluation Report

On 04/24/2024 around 8:00 AM, the California Department of Social Services (CDSS) Community Care Licensing Division (CCLD) Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator, Dwain Carlton JR Griffith. LPA Leandro explained the purpose of the visit and was allowed entry to the facility.

This facility is licensed to serve 6 ambulatory adults ages 18 to 59 years old.

A total of 4 clients are currently residing in this facility.

Facility Layout: The facility is a one-story house located in a residential street. The home consists of: 4 client bedrooms; 1 staff bedroom; 2 full bathrooms; 1 living room; 1 kitchen area with a dining table and an office area; 1 laundry room; 1 attached garage with a gym area; 1 backyard patio area with shaded seating and there is a underground pool that was filled and now there is a small lawn area.
