Facility Evaluation Report
On 3/5/2026, At 8:50AM Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. Upon arrival LPA introduced herself to Blanca Quintanilla- Lead DSP who was informed of the purpose of the visit. Emmanual Sunday arrived approximately 9:10AM and he was informed of the purpose of the visit.
The facility is licensed as an Enhanced Behavioral Supports Home-ARF and are approved to care for (3) non-ambulatory adults age ranges from 18-59 and approved for delayed egress.
The facility is a single-story home located in a residential area. The facility consists of living room, (3) bedrooms, (2) bathrooms, kitchen/family room, activity room and the garage is converted into the staff office/break room.
At 9:57 AM, LPA reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which were current.
At 10:30AM, LPA reviewed three (3) residents’ files for admission agreements, updated physician reports, needs and services plans/IPP all of which were current.
At 12:00PM, LPA conducted a random audit of one (1) residents ‘Medication Administration Records (MARS) which appeared resident have been given their medications as prescribed by their physicians.
At 2:15 PM, LPA and Emmanuel Sunday- Administrator toured the physical plant inside and outside of the facility and there were no obstructions or bodies of water on the premises. LPA observed that the bedrooms have the required furniture which appeared to be in good repair. The rooms also have adequate lighting, and storage for the residents’ personal belongings.
