Facility Evaluation Report
On 07/23/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Annual Visit to the facility listed above. LPA met with Staff Iris Black and the purpose of today’s visit was explained. LPA was granted entry into the facility. The facility is licensed to serve four (4) ambulatory adults ages 18 through 59. The clients are Harbor Regional Center consumers.
Physical Plant/Structure The facility is a single-story structure located in a residential neighborhood. It consists of four (4) client rooms, two (2) bathrooms, living area, dining area, kitchen, outside sitting area, and an attached garage used for storage. All walkways around the facility were observed clean, clear, and free of debris, hazards, and obstructions. There were no bodies of water on the premises. LPA toured the physical plant.
Bedrooms All bedrooms were inspected. Rooms were observed to be containing the required furniture including a bed, dresser, nightstand, chair, and storage space for clients’ personal property. All beds had the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillows. LPA observed an adequate supply of linens in good repair. All bedrooms have adequate lighting.
Bathrooms LPA inspected bathrooms and observed them to be clean and operational. LPA observed the showers have non-skid mats. LPA observed client’s personal hygiene products secured in a cabinet in the hallway and are inaccessible to clients. LPA observed an adequate supply of towels and wash clothes in a cabinet. The water temperature in the bathrooms measured 108-degrees and 108.3-degrees Fahrenheit.
Kitchen LPA inspected the kitchen and found it to be clean and sanitary. LPA observed all appliances are operable. LPA observed an adequate supply of cookware, dishware, and cutlery in good repair. LPA observed a 3-day supply of perishable foods and a 7-day supply of non-perishable foods properly stored. The water temperature measured 111.3-degrees Fahrenheit. All knives and sharps are secured in a locked cabinet in the dining room and are inaccessible to clients.
