Facility Evaluation Report

On 10/03/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – Annual Continuation Inspection and met with Administrator Antonia Dionisio.

The facility is licensed to serve five (5) non-ambulatory residents and one (1) bedridden resident. The facility also has an approved hospice waiver for one (1) resident. The facility currently has two (2) residents receiving home health services and one (1) bedridden resident receiving hospice services. The annual licensing fees are current as of 10/02/2025. The home consists of four (4) resident rooms, two (2) bathrooms, kitchen, dining room, living room, laundry room, attached garage, and outdoor patio area.

Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew, a non-skid mat was in place, and a hot water temperature warning sign is prominently placed in the residents’ bathroom.
