Facility Evaluation Report

On 06/16/2024 around 9:40 AM, the California Department of Social Services (CDSS) – Community Care Licensing Division (CCLD) Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Program Supervisor, Marc Vida and Lead, Gloria de Morales. CCLD staff explained the purpose of the visit and was accompanied by a staff member inside and outside the facility during this inspection.

This facility is licensed to serve 23 non-ambulatory adults ages 18 to 59 years old.

A total of 26 clients are currently enrolled in this facility.

Facility Layout: The facility is a one-story building located on a main street. The building consist of 1 reception area, 1 kitchen / dining room, 2 restrooms, 3 storage rooms, 1 conference room, 1 computer / tv room, 2 activity rooms, and 1 outside patio area with shaded seating.
