Facility Evaluation Report
On 10/05/2021, Licensing Program Analyst (LPA) Don Senaha and LPA Gail Johnson conducted an unannounced case management visit. LPA was greeted by Sina Akai the Program Director and explained the purpose of today’s visit to observe the lighting in the facility. The day program is currently open virtually but has no clients’ attending in person. There were three other staff members present. The facility is licensed for thirty (30) Developmentally Disabled adults of which (30) can be non-ambulatory. The clients ages range from 18-59 and are majority are Harbor Regional Center clients with the exception of (2) clients are from Westside Regional Center.
During the Case Management visit LPA did a complete walk through of the facility. LPA observed 4 bathrooms located in one area, arts and craft room, isolation room/health room, game room, kitchen area, garden room, style room, sensory room, computer room and main activity room in the middle with a television. LPA measured the water temperature of the kitchen sink and two bathrooms to range between 110.6 F and 114.1 F meeting Title 22 regulations.
On 08/06/2021, LPA Senaha received an email from the Program Manager informing LPA that the facility had an external incident involving a truck driver delivering to our neighbor business and ran into the utility box outside of our building affecting the electricity in part of the building.
During today’s visit, LPA observed all electricity for the entire building is working and lighting is not an issue to reopen the day program.
An exit interview was conducted and a copy was left with the Program Director Sina Akai.
