Facility Evaluation Report
On 12/302025 at 1:20PM, Licensing Program Analyst (LPA) Bernadette Allen made an unannounced visit to finish conducting the annual required inspection. LPA met Nancy Cuevas- Compliance Manager.
LPA Allen and Nancy toured the facility inside and outside there were no bodies of water or obstructions on the premises. LPA observed that the clients’ bedrooms had the required furniture, bed, and linen supplies which appeared to be in good condition, there was adequate lighting, and there is storage space for client’s personal belongings.
LPA Also observed additional bed linen and bath towels at the time of visit. The bathrooms appeared to be clean and operational. The water temperature ranged from 105 -120 F° and the facility temperature ranged from 72-75 degrees F.
LPA observed that facilities kitchen to have a 5-day supply of perishables and a 7-day supply of non-perishables food items which were stored and maintained properly. There was also a menu available for review.
Smoke detectors were tested and operating properly and the fire extinguishers were fully charged and the last fire drill was conducted on 12/29/2025.
Based on LPAs observations, review of records and interviews there were no deficiencies cited.
An exit interview was conducted, and this report was discussed and provided to Nancy Cuevas- Compliance Manager. at the conclusion of the visit with appeal rights.
