Facility Evaluation Report
On 12/29/2025 at 2:35PM, Licensing Program Analyst (LPA) Bernadette Allen made an unannounced visit to conduct the annual required inspection. LPA met Nancy Cuevas-Compliance Manager.
The facility is licensed for a capacity of six (6) ambulatory adults ages 18-59.
There are currently six (6) clients enrolled in the program: (6) ambulatory and (0) non-ambulatory. The last medical emergency drill was conducted 11/30/2025. The last fire drill was conducted on 12/29/2025. The facility provides transportation. The liability insurance is current and expires 6/21/2026. The facility fee's are current.
The facility is a one (1) story home located in a residential neighborhood, the property consists of the following: 6 client bedrooms, 2 bathrooms, a staff restroom, nurses office, staff office, living room, kitchen, dining room, attached garage which houses the washer and dryer and an backyard with outdoor shaded area and a therapy room.
At 3:42PM, LPA reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings, which appeared to be current. LPA also reviewed six (6) clients files for admission agreements, updated physician reports, needs and services plans which appeared to be current.
LPA informed Nancy Cuezas- Compliance Manager that due to insufficient time LPA would return to complete the annual inspection.
An exit interview was conducted, and this report was discussed and provided to Nancy Cuevas- Compliance Manager. at the conclusion of the visit.
