Facility Evaluation Report

On 09/26/2025 at approximately 1:00PM, Licensing Program Analyst (LPA) Jose Anguiano conducted a case management visit and met with Administrator Jehn Demafelix. The facility is a one-story, six-bed, three-bath Residential Care Facility for the Elderly (RCFE), currently housing six residents. LPA toured the facility and observed resident rooms A through F, all of which are licensed for bedridden residents. Of the six residents, two are receiving hospice services. Five residents are non-ambulatory, and one is ambulatory. Two residents utilize hospital beds with full rails. During the visit, LPA collected and reviewed the facility’s hospice service documentation, resident register/roster, LIC 500 Personnel Report, and appraisals for two residents.

No deficiencies were observed. An exit interview was conducted, and a copy of this report was provided to the Administrator.
