Facility Evaluation Report
On 07/03/2026, at 12:45pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual visit to Atkinson Home, INC. LPA met with Licensee, Hai Nguyen, and explained the purpose of today’s visit. The facility is licensed to serve four (4) ambulatory clients of which two (2) may be non-ambulatory ages 18 through 59. Hospice waiver for two (2) clients. Currently, the facility has three (3) clients. The clients are Harbor Regional Center and Westside Regional Center clients. None of the clients have restricted health care conditions and none utilizes postural supports or protective devices. The facility’s annual fees were paid during the inspection visit.
The facility is a single-story structure located in a residential neighborhood. It consists of four (4) client bedrooms, two (2) bathrooms, living room, dining room, kitchen, detached garage, and an outside activity area.
At 1:20pm, LPA and staff toured the facility. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, and adequate storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations. Toilets and water faucets worked properly. The shower was free of mold/mildew, there was adequate lighting, and sufficient toiletries were accessible to clients. The water temperature measured 106.5F degrees. A comfortable temperature is maintained in the facility.
LPA observed the facility to be clean, sanitary, and appropriately furnished at the time of visit. Storage areas for cleaning agents, toxins, and sharps were inaccessible to clients. The kitchen was inspected and there is enough perishable and non-peri shable food available for the clients.
