Facility Evaluation Report
On 5/14/25, Licensing Program Analyst (LPA) Alfonso Iniguez conducted a Case Management - Deficiencies visit at the facility. The LPA met with Rosita Lovato/Licensee, and the purpose of the visit was explained.
On 5/14/2025, during the pre-licensing visit at the facility, LPA Iniguez observed unlocked cleaning supplies on one of the bathrooms and under kitchen sink.
Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8. See details below:
-Unlocked cleaning supplies on bathroom and under kitchen sink. Type A citation.

An exit interview was conducted, and a copy of the Facility Evaluation Report was provided to Rosita Lovato / Licensee.
