Facility Evaluation Report
On 08/21/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Annual Visit to the facility listed above. LPA met with Administrator, Demetra Walker, and the purpose of today’s visit was explained. LPA was granted entry into the facility.
Physical Plant/Structure The facility is a single-story structure in a residential neighborhood. The facility consists of four (4) Client bedrooms, two (2) bathrooms, living area, staff work area, dining area, kitchen, laundry area, indoor and outdoor activity area. Outside LPA observed a covered patio with table and chairs available for client use. All walkways around the home were observed clean, clear, and free of debris, hazards, and obstructions. LPA did not observe any bodies of water on the premises.
Bedrooms LPA inspected all client bedrooms and found them to be clean and in good repair. LPA observed all rooms have the required furniture including a bed, dresser, nightstand, chair, lamp, and storage space for personal belongings. LPA observed the beds have the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillows. LPA observed an ample supply of linens, blankets, and comforters stored in the cabinet in the hallway.
Bathrooms LPA observed the bathrooms meet Title 22 regulations. The bathrooms were observed clean and operational. LPA observed showers had nonskid mats. The showers were observed clean and free of mold and/or mildew. LPA observed an ample supply of towels, hand towels, and wash cloths. LPA observed an ample supply of hygiene products for clients. The water temperature in the bathrooms measured 117.8-degrees and 116.3-degrees Fahrenheit.
Kitchen LPA observed the kitchen to be clean and sanitary. LPA observed appliances to be operable and in good repair. LPA observed an ample supply of cookware, dishware, and cutlery. LPA observed a 2-day supply of perishable foods and a 7-day supply of nonperishable foods, properly stored, labeled, and dated. The water temperature measured 118.1-degrees Fahrenheit.
