Facility Evaluation Report
On 02/10/2026, Licensing Program Analyst (LPA), Wendy Gibbs conducted an unannounced Case Management Visit – Incident to follow up on an SOC341 a Report of Suspected Dependent Adult-Elder Abuse. LPA met with Administrator Tanisha McCall, and the purpose of today’s visit was explained. LPA was granted entry into the facility
The SOC341 was submitted to the department, on 01/20/2026 regarding an incident that occurred between two Client on 01/17/2026. On 01/17/2026, Client C2 walked into the living room, after exiting the bathroom, and bit Client C1 on the ear.
During the visit, LPA inspected the facility, interviewed Staff S1 and S2, attempted to interview C2 and received and reviewed C2’s Individual Behavior Support Plan (IBSP) dated 09/2024, Individual-Centered Person Plan dated 10/27/2023, Quarter ISP Review report period 09/01/2025 to 11/30/2025 and staff schedule.
During interviews with Staff, indicated there were two (2) staff on shift when the incident occurred. During interviews staff stated one staff was in the kitchen supervising clients in the living room while the other staff was washing their hands in the bathroom after assisting a Client in the restroom. Client (C2) entered the living room and proceeded to where C1 was sitting and walked up, leaned over C1, and C2 bit C1 on the ear. During review of the Program Design, LPA observed they have a ratio of one (1) staff to two (2) Clients.
During today’s visit, LPA did not observe or cite any deficiencies.
An exit interview was conducted with Administrator, Tanisha McCall, and a copy of this report was provided.
