Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual inspection today. LPA met with Jasmin Junio, Assistant Administrator, and the reason for the visit was explained.
The facility is licensed to serve (6) developmentally disabled adults; ambulatory only. The facility is operating within the scope of its license. The facility is currently providing services to (4) clients through the Eastern Los Angeles Regional Center.
The single-story home is in a residential area of Temple City, and it consists of living room, dining area, kitchen, (2) client bedrooms, (1) staff resting room, (2) full bathrooms, detached garage garage with laundry area, and front and backyard.
The facility was observed clean inside and out. Walkways, passageways and exits are kept clear of debris and obstructions. Furniture is clean and in good repair. Living and dining room have sufficient seating for staff and clients. Temperature was measured in the living room and dining area with LPA's probe thermometer. Temperature in both areas measured 61 degrees F. and area heater also reported temperature of 60 - 61 degrees which is not within the compliance range of 68 and 85 degrees F. The kitchen was observed clean and appliances were operating properly. The facility has a sufficient 2-day perishable and 7-day perishable supply of food. Sharps/knives, cleaning supplies and other toxins are kept locked and inaccessible to clients. Bedrooms have all the required furniture, bedding and sufficient lighting. Bathrooms were also observed clean and sanitary. However, water temperature was tested in bathroom #1 and #2 and measured 99.6 - 101.3 degrees F. which is not within the compliance range of 105 - 120 degrees F.
