The inspector’s account

Licensing Program Analysts (LPAs) Bennette Pena and Daniel Konishi conducted an unannounced Required 1-yr visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA was met by Africa Junio, Administrator and Lodivina Cunanan (Care Staff) and explained the purpose of the visit. The facility is licensed to care for (6) Developmentally Disabled Adults, ages 18 through 59, ambulatory only. All clients residing at this facility receive case management services provided by East LA Regional Center. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Facility has submitted the Infection Control Plan as required by CCL. Infection control practices and Personal Protective Equipment (PPEs) were observed. There is a visitor sign-in station located at the entrance on the the side front door. Staff use disposable gloves to clean and disinfect the high touched surfaces in the common areas. Staff also stated they use gloves when handling food and assisting with medication. Staff are adhering to infection control requirements.

Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood, contains three (3) client bedrooms, two (2) full bathrooms, a living room, kitchen, dining area, backyard, detached garage with additional dwelling unit. Currently, there are four (4) clients living in the facility. Facility is Level 4A. The interior and exterior physical plant was inspected. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, night stand, light, chair and sufficient closet space. Bathrooms have non-skid materials and contained hygiene supplies including liquid soap and toilet paper. Exit doors are free of any obstruction. Backyard was inspected and LPAs observed miscellaneous items and trash. LPA observed an additional dwelling unit connected to the detached garage. LPAs observed kitchen knives, sharps objects, cleaning supplies and toxic substances are not locked and accessible to clients. There is one (1) fire extinguisher observed in the hallway and was last inspected on 02/28/2023, which has just expired. Smoke alarms and carbon monoxide were tested and operable. There is a fireplace in the living room that was not covered. There are no firearms or weapons stored at the facility. There is no swimming pool or body of water observed. Water temperature readings did not measure within the required 105 - 120 degrees Fahrenheit. LPA checked the hot water supply and measured at 100.4 deg F in bathroom #1 and 98.2 deg F in bathroom #2.

Operational Requirements: A current Plan of Operation was reviewed. A fire clearance for six (6) ambulatory clients is in place. Facility cannot provide a copy of a valid Surety Bond Insurance.
