Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management-Deficiencies visit during a complaint investigation today. During tour of the common areas of the facility, LPA observed Client 1 (C1) wearing a helmet inside the home. Interview conducted with Staff 1 and Staff 2 (S1-S2) indicated that C1 wears the helmet due to C1 being a fall risk. LPA reviewed C1’s file and did not observe helmet exception documentation. Further interview with S1 revealed that S1 has not sent a helmet exception request to the department even though C1 has a physician’s order in place for usage of the helmet.
