Facility Evaluation Report

Licensing Program Analyst (LPA) Cota, conducted an unannounced Case Management - Plan Of Correction (POC) visit today to follow-up on pending items discussed during office meeting held on 6/10/2026.. LPA met with Marilyn Acabal, Administrator, and the reason for the visit was explained.

During today's visit, LPA cleared the following citations:

CCR Section: 80061(b)(1)(E) - Reporting Requirements. Proof of training was provided.

CCR Section: 80065(a) - Personnel Requirements. Proof of in-service training was provided.

CCR Section: 80072(F)(1)(2) - Personal Rights. Letter of Support from Regional Center and Care Plan for R1.

Additional items completed today, per agreement with licensee during office meeting:

Administrator provided completed copy of LIC 308 Designation for Facility Responsibility.

Administrator provided proof of Administrator Certificate renewal application sent to the bureau on May 15, 2026 via postal service.

Licensee provided proof of Administrator Certificate #7005199735 which will expire on 6/15/2028.

LPA reviewed relevant documents for Change of Administrator request including LIC 200.

Letter of Support from Regional Center and Care Plan for R1.

Licensee has secured an appointment with Technical Support Program for July 16, 2026 at 10:00 a.m.

No deficiencies noted during visit. Exit interview conducted with Marilyn Acabal, Administrator, and copy of this report and POC Letters of Deficiency Citations Cleared (3) was provided.
