The inspector’s account

Licensing Program Analyst (LPA), Mayra Cota, conducted a Plan of Correction Visit (POC) to follow up on deficiencies cited during complaint visit conducted on 4/2/2026. Upon arriving at the facility, LPA met with Marites Jacinto, Direct Support Staff, and explained the purpose of this visit. On Call Administrator, Mohammed Shirazil, arrived thereafter.

The purpose of today’s visit is to follow up on the POCs that were due on 4/10 and 4/17/2026.

The following citations were cleared today:

CCR Section 80066(a) Personnel Records: Copies of current First Aid/CPR certification observed in S1 and S5's staff file.

CCR Section 80070(a) Client Records: Copies of (5) out of (5) current Quarterly Behavioral Reports and Consultant Logs observed in client files.

Exit interview was conducted with Mohammed Shirazi, On Call Administrator, and a copy of the report and POC letters was provided.
