Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Case Management – Other visit today to address appeals received by the department on 8/7/2025. LPA met with Jacquline Cortez, Executive Director, and the reason for the visit was explained.
The appeals stem from a Required-1 year visit conducted by LPA Cota on 7/17/2025 in which deficiencies were issued for Section 87555(b)(29) General Food Service Requirements and Section 87303(e)(3) Maintenance and Operation.
Review and response to the appeals have determined that the citations mentioned above have been changed from Type A to Type B deficiencies. During today’s visit, LPA will re-issue both citations as Type B on LIC 809-D.
Exit interviews was conducted with Jacquline Cortez, Executive Director, and a copy of the report, 809-D and Appeal Rights was issued.
