Facility Evaluation Report
Licensing Program Analyst, (LPA), Mayra Cota, conducted a Plan of Correction (POC) visit today. LPA met with Jacqueline Cortez, Executive Director, and explained the reason for the visit.
During today’s visit, Civil Penalties were assessed for Sections CCR 87555(b)(27), 87555(b)(29), 87303(e)(3), 87303(e)(4) and 87465(a)(4) for deficiencies issued on 7/17/25 for Failure to Correct by Plan Of Correction (POC) due date of 7/18/25. Civil Penalties which were assessed today for $1000 each deficiency not corrected, total to $5000. LPA Cota, received POCs during today’s visit of 7/28/25 by means of obtaining photo of repair of the grab bar in resident's bathroom, purchase receipt for container lids replaced in the food pantry, photo of water temperature warining sign above the kitchen dishwashing sink, service receipt for pest control conducted on 7/25/25 and in-service training sign-in log which took place on 7/21/25 for medication administration policy and procedures. POCs have now been corrected, per documentation received during today’s visit of 7/28/25.
