Facility Evaluation Report
Licensing Program Analyst (LPA), Tihesha Smith conducted an unannounced Required 1-year inspection at this facility at 10:15 am. LPA Smith rang the ring camera bell, but no one answered the door. LPA Smith observed a dark grey vehicle parked in the driveway. LPA Smith called the facility number at (747) 265-6038 but no one answered. LPA Smith also call the Licensee. LPA Smith then checked to see if there was a different number available online and called the phone number listed at (213) 348-9958 and reached a live person but the call was disconnected due to a bad connection. The individual that answered the call sent LPA Smith a text identifying himself, as Ryan, a care specialist with Victory Bay from New Jersey. LPA Smith replied that she is with The Dept of Social Services and requested to have the licensee of the facility contact her back. LPA received a call back from the Licensee, Kiersten Pennington, at approximately 10:40 am and disclosed the purpose of the visit. The licensee stated that the staff and clients are on an outing and would arrive later. The Licensee stated she would be able to come to the facility in approximately 30 minutes.
LPA Smith and the licensee toured the facility to ensure there are no health and safety hazards, and the facility is following Title 22 Regulations at approximately 11:30 am. Common areas, including the living room, dining room, kitchen, and the recreation room were evaluated for their ability to safely meet residents' needs. These spaces were inspected for cleanliness, and furniture was assessed for functionality and condition. All common areas were found to be clean, sanitary, and furnished with seating sufficient to accommodate residents comfortably. LPA reviewed the food service areas, food storage and supply (perishable and nonperishable foods).
