Facility Evaluation Report

Licensing Program Analyst (LPA) Jose Tan conducted an unannounced case management visit at this facility to follow up on the incident that occurred on 03/19/26 where the live in Administrator or Staff #1 (S1) passed away in the facility. LPA was greeted by the Licensee Representative Eric Roberts and explained the reason for the visit.

LPA conducted physical plant tour at 9:45 AM, reviewed records between 9:45 AM to 10:15 AM and interviewed the administrator and the licensee between 10:15 AM to 11:00 AM.

LPA observed that the room of the former Administrator was sealed off and no one was allowed entry. LPA confirmed that no residents are currently at the facility. Per the Administrator, they moved last night (03/24/26) to an Air BnB location in Newhall, CA until the room is cleared of Biohazard materials, in coordination with the North Los Angeles Regional Center (NLARC). Further, the clean-up will take a day or two (2) maximum but they would rather stay until the S1’s family member come to pick up S1's belongings on 04/01/26 coming from out of state.

Further interview also revealed that there was no any indication that S1 would pass prior to the incident. The facility offered grief counseling to all residents and staff and will continue to have sessions today and up to whenever necessary.

There were no health and safety and personal rights issues during this visit.
