LPA conducted an unannounced case management visit at this facility to issue citation for the deficiency discovered during the course of the investigation of complaint control number 31-AS-20250609010348 wherein the staff working at the facility did not have fingerprint clearance and per interview with staff, staff had been working at the facility for two (2) weeks.
