Facility Evaluation Report
On Thursday, 5/07/26, Licensing Program Analyst, (LPA) Ray Comer, arrived to conduct an unannounced annual inspection. LPA Comer met with the Program Director, and reason for the visit was discussed.
The facility accessed by two (2) main entrances/exits, and is comprised of an Adult Day Program. The property consists of one large building, which includes the administration office area, a nurse station, and reception area. Required postings were observed in the office area. Facility has six (6) gender neutral bathrooms and six (6) activity rooms.
At 10:25 AM, LPA conducted a tour of the physical plant and observed the following:
Kitchen: At 10:45 am, LPA observed the facility's kitchen, which was locked. Facility provides hot meals, snacks, and drinks. Kitchen was fully stocked and operational. Knives and sharps were secured in locked kitchen cabinets, and inaccessible to clients.
First Aid: First-aid kit was located in the nurse station, observed as fully stocked with manual .
Fire detection system: is present in the facility. Facility has a sprinkler system; LAFD Reg. 04 test, conducted on 3/9/26, shows all systems as "passed" inspection. Dual smoke\carbon monoxide detectors are hardwired, and interconnected throughout the facility; detectors were tested and functioned properly. Eight (8) fire extinguishers were observed by LPA to be fully charged; all were inspected on 06/15/2025. Emergency Fire Drills were recently conducted on 04/02/2026.
