Facility Evaluation Report
Licensing Program Analyst (LPA) Abeye Duguma met with Teresa Rathsam for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 09: 00 AM and the following was noted:
This is an Adult Day Program dedicated to empowering individuals with developmental disabilities through inclusion and personal achievement in the community. Facility offers volunteering opportunities and other workforce development programs. The facility is fire cleared forty (40) non-ambulatory. The facility is currently providing Day Program services to fifty-five (55) clients, both on site and community based. Facility has multiple rooms for clients to participate in a wide range of activities from the arts to small businesses such as cafés and kiosks.
All rooms and hallways are well lit. The facility is neat and clean. The bathroom was checked for cleanliness and proper operation. Clients have enough hygiene products provided by the licensee. The facility has a swimming pool/body of water with fencing, locking mechanism and on-duty lifeguard when in use.
Facility prepares breakfast and lunch for clients. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests.
