Facility Evaluation Report
Licensing Program Analyst (LPA) Tuesday Cabiness arrived at the facility to conduct an unannounced infection control inspection/visit. Upon arriving, LPA was greeted by staff, who escorted LPA to the Administration office. LPA’s temperature was done with an electronic thermometer. LPA was asked a series of health screening questions, pertaining to COVID-19, and signed in the visitor book. Administrator Areyah Rosner was not available, but Co-Administrator Esther Meraz conducted the inspection with LPA. According to Esther, there have not been any active or past COVID cases at the facility in the past year. All staff and clients are vaccinated, and booster shots are scheduled for next month. The current census is (92). LPA observed staff and some clients to have full mask covering. There are hand sanitizing stations located by the front office, medication room, hallways, and dining room. COVID-19, CDC, Department of Public Health, and Licensing postings and were visible seen and posted on the walls throughout the facility.
The infection control inspection began with the Co-Administrator Esther, who escorted LPA throughout the facility. The facility has three (3) buildings, all were inspected. Client's bedrooms appeared to be clean and appropriately furnished for the comfort and safety of clients. Beds were (6) feet apart. Passageways were free and clear from obstruction. Inside temperature was maintained at a comfortable level. All common areas were observed to be clean, including client rooms, and staff and visitor bathrooms. Soap and towels, and hand washing signs were visually posted. The facility has cleaning procedures and protocols in place, which include staff and housekeeping cleaning common areas, and doorknobs. Currently, the facility continues to require mask wearing; but it is not mandatory.
Masks are provided when needed by the Administration office. Temperature checks are done in the morning and when staff or clients display any symptoms. There is also a sink outside for clients and staff to wash hands prior to entering the facility
