Facility Evaluation Report
Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an annual required visit and inspection of the facility. LPA met with Rebecca Smith and explained the reason for the visit. At 10:52 AM, a tour of the physical plant was conducted and the following was noted:
There is one entrance being utilized at the facility. The facility has a total of eight (08) bedrooms and four (04) bathrooms. Clients bedrooms were properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. The facility is fire cleared for sixteen (16) ambulatory. The facility is currently occupying eight (8) residents.
The facility has a lobby and an intake office near the front entrance for residents and visitors. The facility does not have a swimming pool/body of water.
All meals are catered and served. Kitchen area is sufficiently stocked with snacks and beverages. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. The facility does not keep sharps and knives on the premise. Garbage cans have tight fitting covers. Cleaning supplies, pesticides or toxic cleaning supplies were stored and locked away in the laundry room.
The common and activities areas are neat and clean. The facility maintains a comfortable temperature at 74°F. The smoke and carbon monoxide detectors are hardwired, interconnected and centralized. A signal is automatically dispatched to the local fire department. Fire extinguisher is located at the west end of the facility and observed to be fully charged and last inspected 1.13.2026.
