Facility Evaluation Report

In conjunction with complaint# 31-AS-20250630142546 , LPA Leslie Ngo-Castaneda conducted a case management deficiency. During the complaint investigation, LPA conducted a facility a facility tour at 9:20 AM and the following was found S2 medication in an unlocked kitchen cabinet and was also observed in the living room unlocked drawer. LPA observed one of R3 medications was at the living room beside the sofa by the office. Medication cabinet in the kitchen was also kept unlocked where facility staff put a tape on the latch to keep it unlock.

Exit Interview Conducted. An immediate civil penalty cited. Report Issued.
