Facility Evaluation Report
Licensing Program Analyst (LPA) Tihesha Smith and conducted a case management for deficiencies in conjunction with complaint # 31-AS-20250514161807 to address a deficiency observed during complaint visits. LPA Smith disclosed the purpose of the visit.
On 10/15/2025 and 11/20/2025 LPA Smith observed the pantry and dining room cabinet storing foods and/or canned goods were in accessible to residents in care. Staff reveal items locked to prevent residents from eating or drinking items purchased for specific residents.
Based on the above observations and interviews deficiency cited on LIC 809-D.
