Facility Evaluation Report

Licensing Program Analyst (LPA) Tuesday Cabiness and Licensing Program Manager (LPM) conducted a case management visit in conjunction with complaint control numbers: 31-AS-20250529125532 and 31-AS-20250529104139.

During today’s visit, two (2) staff members were observed working on duty who were not fingerprint-cleared or associated with the facility . LPM verified both individuals using the Department’s internal system (LIS), and neither name appeared on the Personnel Summary Report .

As a result, civil penalties will be assessed in the amount of $500.00 for each staff member , totaling $1,000.00 , due to the facility’s failure to ensure staff were properly cleared and associated prior to beginning work.

An exit interview was conducted , the citation was issued , civil penalties were assessed , appeal rights were discussed , and a copy of the report was provided to the facility.
