Facility Evaluation Report

On 10/06/25, at 10:30am, Licensing Program Analysts (LPA)s Nadia Shahbazian and Tuesday Cabiness arrived at the above facility to conduct an unannounced case management visit for the purpose of closing the facility and cease operation. LPAs met with Hayle Jimenez-Director of Operations and explained the purpose of the visit.

CCLD was notified of the intent to close this facility, but no specific date of closure was given. An application was submitted to Community Care Licensing Division- CCLD on 07/14/2025 for change of location. LPAs were unable to do a walk through, since Director of Operations stated the keys were relinquished to the owner already. LPAs bserved no evidence that currently the facility is being operated as a Social Rehabilitation Facility. Director of Operations relinquished the facility license to LPA. No deficiencies cited during this visit.

An exit interview was conducted and a copy of the report was given.
