Complaint Investigation Report
A civil penalty was assessed in the amount of $500 for fire clearance violation. Therefore, this allegation is Substantiated.
Allegation #2: The facility is exceeding the maximum bedroom capacity. During the investigation conducted on June 4, 2025, and June 5, 2025, between 9:30 a.m. and 2:30 p.m . , LPA conducted staff and resident interviews, reviewed facility documents, and completed a physical plant inspection. It was alleged that the facility housed more than two (2) residents per bedroom. It was confirmed and reported to LPA that room #1 previously contained three (3) beds, which were later relocated to a downstairs area that had been designated as a staff office. During the inspection, room #1 was observed to have only one (1) bed and one (1) client, with the two additional beds having been removed. Although the beds were relocated, based on interviews, documentation, and prior room usage, there is sufficient evidence to confirm that the facility had previously exceeded the approved bedroom capacity. Therefore, this allegation is Substantiated at this time. Note, citation issued for this violation, will be corrected during visit, due to the facility removing the (2) additional beds.
