Complaint Investigation Report

Based on interviews and the information obtained, several residents stated that they were not aware that toilet paper is not provided. Most residents were not concerned, since they were already purchasing their own personal supplies. Only two residents interviewed, mentioned that they have been provided toilet paper by the facility staff, upon asking and there should not be additional charges added to their rent. Based on review of admission records for residents, LPAs verified that personal supplies are listed as responsibility of residents, therefore, the allegation is deemed Unsubstantiated at this time.

An exit interview was conducted, and a copy of this report was given.
