Facility Evaluation Report
Licensing Program Analyst (LPA) Sakinah Madyun conducted an unannounced continued Annual Inspection visit at the facility using the CARE Inspection Tool.
During today's continued annual visit LPA observed the following:
Infection Control : LPA reviewed Infection Control Plan dated 7/16/25.
Disaster Preparedness : LPA reviewed Emergency Disaster Plan dated 2/7/25. Emergency Drills are conducted monthly and the last emergency drill was conducted on 6/20/25. Facility has fire extinguishers and fire sprinkler system throughout as well.
Staffing: The facility has sufficient staffing and present during the program.
Staffing- Administrator Karissa Tressa meets the education requirements per Title 22 Regulations. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility.
Personnel Records-Training- Staff files are maintained and centrally stored at the facility. LPA observed training records and CPR and First Aid certifications. LPA reviewed (5) staff files and observed TB results, Health screening, fingerprint clearance and job application. All the staff files reviewed are within Title 22 requirements.
Inspection tool completed. No deficiencies are being cited today. Exit interview conducted with Amber Baldwin and a copy of this report was provided.
