Facility Evaluation Report

Licensing Program Analyst (LPA) Tena Herrera conducted a case management visit to issue a deficiency observed during the investigation for complaint # 28-AS-20251022115757 . LPA met with Assistant Administrator Stacey Cheat and explained the reason for the visit.

During the complaint investigation, LPA discovered that Administrator Vanessa Vasquez was terminated on 9/25/2025. LPA reviewed all documents that have been faxed/emailed to the department and there was nothing received stating that Administrator Vasquez is no longer at the facility. Interview with Assistant Administrator Stacey Cheat revealed that they have been assisting facility with Administrator duties during the absence and search for replacement. Per regulation 85064.3 Administrator Recertification Requirements (j) Whenever a certified administrator assumes or relinquishes responsibility for administering an adult residential facility, he or she shall provide written notice within thirty (30) days to: (1) The local licensing office(s) responsible for receiving information regarding personnel changes at the licensed facilities with whom the certificate holder is or was associated. Therefore, a citation is being issued and detailed on the LIC809-D page.

Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiency observed during the visit are documented on 809D. Exit Interview held and copy of the report and appeal rights were provided to Assistant Administrator Stacey Cheat .
