Facility Evaluation Report
Licensing Program Analyst (LPA) Tena Herrera made an unannounced Case Management visit to the facility. LPA met with Program Director Lauren Davis and explained the reason for the visit.
On 7/24/25 the Department received a Special Incident Report (SIR) for an incident that was discovered by the facility on 7/23/25, in which Staff #1 purchased and EBT card belonging to Client #1. The transaction involved S1 giving C1 money for their EBT card and Pin information on 3 occasions, facility staff conducted an internal investigation and S1 admitted to purchasing C1’s EBT card. Per administrator the funds and card have all been returned to C1 and S1 has since been terminated from working at the facility.
Per California Code of Regulations, Title 22, and California Health and Safety Code, there will be a deficiency issued during today’s visit and will be documented on the LIC809-D page.
