Facility Evaluation Report

Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced visit for the purpose of conducting the required annual inspection. On today's visit LPA met with program manager, Evelyn Prado who assisted with the visit. Facility has 34 in-person clients and 14 alternative clients.

The facility is vendorized through South Central Regional Center and is licensed to served 66 clients between the age of 18 and above, ambulatory only. The facility has 2 single story buildings and a garage (Licensee office). Building #1 contains 1 classroom and 1 restroom. Building #2 contains 1 classroom, 2 restrooms, office, quiet area, and a kitchen.

LPAs discussed infection control practices with program manager and director, toured the facility inside and out, reviewed food supply, reviewed staff files, and reviewed resident medications.
