Complaint Investigation Report
LPA Arroyo conducted additional staff interviews on 02/25/2026 at 02:00 p.m. and 02:15 p.m. and on 03/13/2026 at 11:20 a.m. On 03/24/2026, LPA Arroyo conducted a collateral visit at Simi Oaks Counseling Group and conducted interviews with two clients at 11:45 a.m. and 12:10 p.m.
It was alleged that staff speaks inappropriately to clients in care. It was reported that staff have been observed threatening violence toward clients and using derogatory language. Interviews conducted with staff revealed that some staff has been observed speaking to clients in a stern manner. Additionally, six out of eight staff interviewed denied observing any staff speak inappropriately to clients, and all staff interviewed denied speaking inappropriately to clients at any time. Interviews conducted with clients revealed that staff treat them well and expressed no concerns with how staff speak to them. During these interviews, three out of four clients denied ever being called names or observing staff call other clients derogatory names. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations “staff speaks inappropriately to clients in care” is deemed Unsubstantiated at this time.
It was also alleged that staff is sleeping in client's bed. It was reported that staff have been observed sleeping in clients’ beds after their scheduled shifts. Interviews conducted with staff revealed that some staff has been observed resting in the living room after disclosing that they were not feeling well; however, they denied observing any staff sleeping in a client’s bedroom. Additionally, all staff interviewed denied personally using a client’s bed to sleep or take a nap. Furthermore, during client interviews, three out of four clients denied observing any staff sleeping or napping in their own bedrooms or in other clients’ bedrooms. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations “staff is sleeping in client's bed” is deemed Unsubstantiated at this time.
It was further alleged that staff are controlling how clients are spending their money violating their financial rights. It was reported that staff have been observed dictating and controlling how clients are allowed to spend their own money.
