Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi arrived at the facility unannounced for a required one-year annual inspection today. Upon arrival, LPA met with Program Coordinator Riley Breen and explained the reason for the visit. At approx 10:30 a.m., toured the physical plant to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The following was noted during the plant tour:
The day program operates from 9:00 a.m. to 2:00 p.m. The day program is currently staffed appropriately with the necessary staff to consumer ratios of 1:3. Common Activity Space: The facility is a two-story structure. Downstairs contains activity rooms, a conference room, two (2) bathrooms, office, and a food service area. The second floor includes administrative offices, activity room, one (1) bathroom, and a conference room. The LPA did not observe any obstructions or hazards. Fire extinguishers were charged and serviced 06/11/2025. Fire safety systems are checked bi-annually.
Kitchen appliances appeared clean and were in operable condition at the time of the visit. Knives and sharps were observed in a locked cabinet. Equipment used for activities was in good condition at the time of the visit. Carbon monoxide detectors are wired and appeared to be operational at the time of the visit. No bodies of water noted at the time of the visit. Activities are both designed for both as individuals and as a group. The LPA observed staff interacting with individuals during time of visit. No obstructions or hazards were observed inside or out.
Restrooms were clean and sanitary. Restrooms were observed with a cabinet that included disinfecting wipes and disinfecting spray which were observed secured at the time of the visit. The LPA observed trash cans with lids to protect individuals from cross contamination.
