Facility Evaluation Report

On 07/25/24, Licensing Program Analysts (LPA), Wendy Gibbs and Deborah Lee, conducted an unannounced Annual visit to the facility listed above. LPA met with Administrator, Josephine Santos, and the purpose of today’s visit was explained. The facility is licensed to served 45 adult clients. The program hours are from 9am-2pm. During the visit, there were no clients present.

Physical Plant/Structure The facility is a single-story building located in an industrial neighborhood. At the entry of the facility is a reception area where you check in. Behind the reception area are staff offices and a conference room. Rooms for client include Computer Area, kitchen area, style shop, music room, relaxation room, art and craft room, game room, exercise room, and activity room. Each room is equipped and furnished to ensure the comfort and safety of all persons in the day program. The is facility maintained a comfortable temperature. Outside LPAs observed a shaded area with table and chairs. Additionally, LPAs observed a basketball hoop and other outside activities available for client use. LPAs did not observe any bodies of water on the premises. All restrooms were clean and operational. The water temperature, in the bathrooms and kitchen, measured between 105-degrees and 107-degrees Fahrenheit. All rooms and hallways were observed with ample lighting. All walkways inside and outside of the
