Facility Evaluation Report

On 8/7/26 Licensing Program Analysts (LPA) Deborah Lee conducted an unannounced annual inspection visit, LPA met with Program Director Summer Nguyen as the purpose of today's visit was explained. The day program is licensed to serve 30 non-ambulatory developmentally disabled adults, consumers are linked to the Harbor Regional center. The staff to consumer ratio is 1 staff to 2 consumers. There are currently 39 consumers enrolled, 25 consumers attend the morning session, and 17 consumers attend the PM session. There are currently 9 consumers who have restricted health conditions, Day Program fees are current.

The day program is a single-story building located in an industrial area. The day program consisting of the following: activity/game room, computer room, art room, style room, nurse station, sensory room, 2 bathrooms, a changing room, kitchen, storage/lockers, and administrative offices.
