Facility Evaluation Report
On 7/31/25 Licensing Program Analysts (LPA) Villegas conducted an unannounced annual inspection visit, LPA met with Program Director Summer Nguyen as the purpose of today's visit was explained. The day program is licensed to serve 30 non-ambulatory developmentally disabled adults, consumers are linked to the Harbor Regional center. The staff to consumer ratio is 1 staff to 3 consumers. There are currently 51 consumers enrolled, 30 consumers attend the morning session, and 21 consumers attend the PM session. There are currently 14 consumers who have restricted health conditions, and 8 clients with a g-tube. Day Program fees are current.
The day program is a single story building located in an industrial area. The day program consisting of the following: activity/game room, computer room, art room, style room, nurse station, sensory room, 2 bathrooms, a changing room, kitchen, storage/lockers, and administrative offices. The facilities kitchen observed to be clean and well maintained, consumers bring their own meals and program provides snacks only. The last disaster drill was conducted 07/10/25, the last inspection held by the fire department was on 11/6/24, (2) Fire extinguishers were properly charged, and the first aid kit was available and fully stocked. LPA observed sufficient furniture and lighting throughout the facility, walls and floors were in good condition, walkways throughout the day program and all exits were clear of hazards and debris.
LPA reviewed 5 consumer records, and 5 staff records, no discrepancies observed. There are 10 consumers with prescribed PRNs, there is a LVN (VN 247879) on premises that assist with administrating medications as prescribed.
During today’s visit no discrepancies were observed.
