Facility Evaluation Report

On 10/06/25, at 9:05am , Licensing Program Analyst (LPA) Gina Saucedo arrived at the above facility to conduct a case management visit with Diana Kenez, Licensee for the purpose of closing the facility and cease operation. The Licensee in July of 2025 requested to close the facility due to personal reasons. LPA conducted a walk-through of the above facility to ensure there were no residents and to officially close the facility. LPA observed no evidence that the house is being operated as a care facility; no deficiencies cited during this visit. LPA Saucedo will submit the file for closure upon returning to the Woodland Hills South Regional Office.

LPA advised licensee to surrender the facility license certification as a part of closing facility and the license certification was provided to LPA.

The report was signed during the visit, and a copy of the report was provided to the licensee.
