Facility Evaluation Report
Licensing Program Analyst (LPA) Zabel Chochian arrive to this facility today to conduct a required annual inspection. Prior to conducting the visit, LPA contacted Tiffany Naumann Chief Clinical Officer. LPA was informed that the facility closed back in 2/2025 and the license was surrendered. Mrs. Naumann stated that their lease was up a year ago and they did not renew the lease for this property since 2/2025. Closure process was discussed. LPA explained that a visit is required before closing the facility. Mrs Naumann stated that the property is most likely rented/leased already. Mrs. Naumann agreed to sign the final report following LPA's site visit.
Upon arrival to the property LPA observed the property to be gated with two vehicles parked in the driveway. LPA was able to speak with the tenant who stated that they leased the property back in 6/2025. Tenant confirmed that the home is occupied as a single family home and they are not operating any kind of business. LPA was not allowed to enter the property. LPA did not observe any sign of a business or unusual activity at this property at the time of visit.
LPA spoke with Mrs. Naumann, Chief Clinical Officer and was informed that they are no longer operation a business at this property and requested that it be closed.
