Facility Evaluation Report
Licensing Program Analyst (LPA) Erica Mosley conducted an unannounced Collateral inspection at the above listed facility at 9:50 a.m. due to a complaint unrelated to this facility. LPA was greeted by staff who called the Administrator to inform them of the reason for the visit. LPA met with Angela Krbashyan , Licensee representative telephonically and the reason for the visit was explained. Entrance interview conducted.
During today’s visit LPA conducted a physical plant tour to ensure there are no immediate health and safety concerns and interviewed staff #1 (S1).
Exit interview conducted. A copy of the report was reviewed and provided.
