LPA Abeye Duguma (LPA) attempted to conduct a Required One (01) Year inspection of the facility but to no avail. LPA arrived at around 11:35a.m., there were no cars in the driveway or any signs of activity. LPA approached the front door and rang the door bell. When no one answered LPA rang the door bell again but still no one came to the door. LPA called the facility three times, no answer.
The attempted visit will be recorded and LPA will return another day.
Nothing further at this time. A copy of the report and business card were left at the facility.
