The inspector’s account

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Sonny Tan and Melita Cabarles, and explained the reason for the visit.

At approximately 8:39am, with the assistance of staff, LPA took a tour of the physical plant. The facility is a two story building. The first floor only, carries the license for the Residential Care Facility for the Elderly (RCFE). The second floor, is living quarters for staff and family only. No resident access. Required postings were observed in the open by the kitchen area. The smoke alarms and carbon monoxide are hardwired and interconnected. The fire extinguisher is located in the dining room. It was charged on December 27, 2025.

Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives are stored and inaccessible to the residents. Cleaning supplies are kept in the garage.

Bedrooms: There are four (4) bedrooms, three (3) designated for residents' use, and one (1) for staff. Bedrooms, in use by the residents are shared. Bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting.

Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures, grab bars and non-skid mats. Hot water temperature was measured between 115 and 117 degrees Fahrenheit. No cleaning supplies were observed in either bathrooms during the day of the inspection.
