Facility Evaluation Report
Licensing Program Analyst Abeye Duguma (LPA) conducted an unannounced Case Management Visit to the facility. LPA met with administrator Tammy Doyle and explained the reason for the visit.
The Regional Office received an incident report regarding Client #1's (C1) Death January 3, 2026. LPA requested Physician's Report, Care Plan and Medication Administration Records. It was reported C1 took a bite, looked at staff like they were going to say something, then slumped over and fell to the floor. CPR and Defibrillator used. Emergency services immediately contacted and responsible parties notified.
On January 14, 2026 at around 11:30a.m., LPA interviewed administrator Tammy Doyle. Tammy stated she was not there and official cause of death was not known to facility.
On January 15, 2026 at around 1:30p.m., LPA interviewed three additional staff and requested additional documents.
No health and safety hazards noted during the visit.
Exit interview conducted and a copy of the report issued.
