On 12/16/2025 at approximately 09:30 AM Licensing Program Analyst (LPA) Troy Watson conducted an unannounced case management visit regarding complaint # 11-AS-20251209121524.
The investigation between 09:30AM - 12:25PM the department met with the Administrator Nathan Venzon and during the visit the department requested the following records : Staff Roster 12/16/2025, Personnel Report 12/16/2025, Residence and Care Agreement 09/24/2025, Electronic Observation Chart 11/09/2025.
An Exit interview was conducted with the Administrator Nathan Venzon and a copy of this report was provided.
