On 9/15/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced visit to this facility. LPA was met by the Director of Resident Services, Brook LaMotte and explained the purpose of the visit is to follow-up on complaint #11-AS-20250429151406. LPA was granted access to the facility.
LPA Felisa Shirley interviewed Resident 4, 6, 7, and 8. Resident R-2 and R-9 were not available for interview.
An exit interview was conducted with the the Director of Resident Services, Brook LaMotte and a copy of this report was provided.
